The Mental Health Association purchases psychiatric medication for persons in Okaloosa and Walton Counties who cannot afford to pay for prescriptions. The Association has provided this community service in collaboration with Sam's Club Pharmacy since 2005. The MHA purchases a
one-month supply of prescriptions, on a one-time basis,
upon request of a healthcare provider verifying that no other source of funding is available.
Information needed: |
- Client Name & Date of Birth
- Diagnosis
- Name of the doctor that wrote the prescription, & dosage
- Reason for need (unemployment, no insurance, meds not covered, etc.)
- Who will be doing follow-up therapy?

Once the request for assistance is received at the MHA by a phone call and approval for payment is determined, the client takes their written prescription to the approved pharmacy (Sam's Club Pharmacy in Fort Walton Beach or Wal-Mart Pharmacy in Defuniak Springs). It's that easy! Call the MHA at 244-1040 for assistance.
